All other occupational and professional listings must be requested through Promissor at (888) 204-6192. Promissor's address is, Department DC-FD, Metro-Plex II, 8201 Corporate Drive, Suite 400, Landover, MD 20785.

Form

You must fill out a License Information Request Form*, available on this site as a PDF document, for all license information requests. The form must be signed and dated by the customer before it is considered a valid request for information. Customers requesting data on electronic media should send diskettes or Zip disks along with the form or an additional charge will be made. Files may be downloaded to an FTP site or transmitted via email at no additional charge.

Fees

All fees must be collected with the written request and before any work on the request is started. Checks and money orders are to be made payable to THE DC TREASURER and properly dated and signed. Telephone requests may be taken, but the requirement for a written request stands. For a current list of license fees please click here.
